PROVIDENCE, R.I. (WPRI) — The Rhode Island Attorney General’s Office has identified a woman who died in state care.

According to our news partners at the Providence Journal, the woman was 70-year-old Barbara Annis.

Annis was a resident of the College Park Apartments on Mount Pleasant Avenue. She suffered a broken leg and was not set for outside care for days after her injury.

After Annis’ death and several reports of abuse within the group home, the RI Department of Behavioral Healthcare, Developmental Disabilities and Hospitals announced the facility would be closed.

The doors were closed for good on March 25 and all 14 residents were transferred to other facilities.

Five employees have been placed on paid leave pending investigation.
