PROVIDENCE, R.I. (WPRI) - The Rhode Island Health Department Tuesday announced the location of its H1N1 flu vaccination clinic for Rhode Island students who attend school out of state.
The clinic will be held Dec. 12 at the Cannon Building in Providence, which is the Health Department's headquarters and is located at 3 Capitol Hill, just north of the State House.
The clinic will run from 10 a.m. to 2 p.m. Parents wishing to have their children vaccinated during the event must pre-register by Dec. 2, health officials said.
An online enrollment system is available, but will close Wednesday at 4 p.m.
Eligible children must bring a signed and dated consent form in order to be vaccinated.
Children who missed their school-based clinic, non-Rhode Island residents, preschool students and college students are not eligible for the event, health officials said.
Those students who missed their school clinic will need to be vaccinated in their pediatrician's office when enough vaccine becomes available.
