Providence Mayor David Cicilline made an appeal to parents Friday morning to do the necessary paperwork so their kids can get the H1N1 vaccines at school clinics.

Both the mayor and the Rhode Island Department of Health are urging parents to make sure to get the permission slips home, signed, and back to the necessary people at school so all the Is are dotted and Ts are crossed.

"It is very important, that if parents intend for their child to be vaccinated, that they sign a consent form, because no child who doesn't have a consent form signed by a parent will be seen. Without that consent form, the department will not provide the vaccination," said the mayor at Friday's meeting at Central High School.

The school-based clinics begin rolling out statewide on November 2. To find out when your child's school will provide vaccinations, check out WPRI.com's full list from the DOH.
